Programs & Services - Home Health Services

CARES provides both custodial home care services and skilled home care services to help elders maintain their independence and remain safely in their homes. For more information or to request any of these services, call (727) 868-3285 in West Pasco or 1-800-833-3285.

Companion:

Provides companionship and escort service e.g. to doctors' appointments.

Home Health Aides:

Home Health Aides provide personal care and respite for caregivers.

Homemaking:

A trained homemaker assists with light housekeeping, shopping, and laundry. This program allows elders who require some assistance to remain in their own homes and participate in the activities of daily living.

Honeywell HomMed Monitoring and MedPartner:

This system allows individuals to maintain their independence while at the same time getting necessary medical and medication monitoring. Each day their vital signs are transmitted for review by registered nurses in CARES Home Health Department. With the MedPartner, individuals are reminded to take their medications at the correct time.

Therapies:

Occupational, physical and speech therapies are available

Skilled Nursing Care:

is provided as needed.

Weekly Medication Set Up:

To insure that patients take their medications, nurses measure out the required doses for the week.

Health Management:

Nurses visit patients to monitor their health status
