Programs & Services - Care Management

CARES Care Managers plan, coordinate and monitor the delivery of services available under a variety of programs. For information or to apply for any of the following subsidized programs, call the Area Agency on Aging of Pasco-Pinellas, Inc. Senior Helpline: 1-800-861-8111

Aged/Disabled Medicaid Services:

This program provides Medicaid services to eligible persons who would otherwise need nursing home care. A variety of in home and community based services are available to enable participants to remain in their homes or in the home of a relative or other responsible person.

Alzheimer's Disease Initiative:

This program provides services to meet the special needs of individuals aged 18+ with Alzheimer's disease and related memory disorders. Clients and caregivers receive a comprehensive assessment of their needs and of their eligibility to receive services. In home or facility based respite and training for caregivers are provided.

Assisted Living Services:

This program provides supplemental payments to specially licensed Assisted Living Facilities to allow eligible individuals to remain in that type of facility instead of a more costly nursing home setting. Care management is provided to insure the continued appropriateness of the arrangements and to monitor client need.

Community Care for the Elderly:

This program provides services to assist functionally impaired older individuals to remain at home. Services are arranged for and coordinated by Case Managers and Case Aides. Eligibility: 60 years of age or older and assessed as functionally impaired and at risk.

Consumer Directed Care:

Medicaid recipients who are enrolled in this program are allowed to exchange their traditional home and community based services for a budget equivalent to the estimated value of the services they had been receiving. With the support of a trained consultant and a bookkeeping and tax service, consumers develop plans of care and hire their own workers.

Home Care for the Elderly:

This program helps caregiver(s) maintain low-income seniors in their own home or in the home of a caregiver. The caregiver receives a monthly basic subsidy payment to assist with the general cost of providing care. Special subsidies that provide for purchases of special supplies, equipment, or services needed to maintain the health and well being of the elderly person may be available. Eligibility: 60 years of age or older, at risk of nursing home placement and financially eligible.
