Medicare is no charge with or without a supplement. Just copy the front of your Medicare card and front and back of Insurance card and bring to the center together with the Health Department form that you picked up when you signed up. Please fill out the form at home and bring it with you so that there will be less waiting. Cards may also be copied at the Center ahead of time, 10 cents per copy.
HMOs need not copy cards. For HMOs, the flu shot is $25, pneumonia is $45, both reimbursable from your HMO with receipt. Tetanus is $32 for all.
