CARES Adult Day Health Service Centers provide
a full day of activities and medical monitoring in a friendly supervised
environment. Individualized care plans are established in coordination
with physician orders. Adult Day Services also provide respite or
an option for the caregiver who must work and cannot leave the patient
at home unattended. Who Should Attend CARES Adult Day Services? Any individual who wants to be a part of their community and remain socially active, but are challenged by a decrease in physical, cognitive, medical or functional abilities. Therapeutic Activities CARES Adult Day Services differ from other forms of care in its unique focus on the strengths and abilities of an individual with functional impairments rather than on their limitations and illness. Medical Monitoring
CARES Adult Day Health Centers have licensed Florida Nurses on
staff. Center nurses monitor the health conditions daily of each
participant. Admission Criteria CARES Adult Day Health Services use a range of tools to assess an individual’s physical, cognitive, medical and functional status. A patient must have the Adult Day Health Service Physician Order form completed within 45 days prior to the admission date.
