HOME CARE AND HOMEMAKER SERVICES

These programs help older adults remain safely in their own homes.  For those who need help at home, the custodial home care services include care management, personal care, nursing service, medical transportation, an emergency alert system, home delivered meals, medication management and therapies.  The homemaker services include light housekeeping, shopping, and laundry.  Services are available to adults 60 years of age and older.  Applicants are assessed to determine their eligibility to receive services.

 

HONEYWELL HOMMED HEALTH MONITORING SYSTEM

The Honeywell HomMed Health Monitoring System allows individuals to maintain their independence while at the same time getting the medical monitoring that they need.  HomMed users have the assurance of knowing that every day a CARES health care professional is monitoring their health.  The Honeywell HomMed System is easy to use and takes less than three minutes to collect and send an individual’s vital signs for review by CARES registered nurses.  If the data indicates a need for intervention or if the user fails to collect their vital signs when prompted, appropriate action is taken.  .

OPTIONS

CARES Options Program is a private pay service (including insurance) that provides services to adults of     any age on either a long term or short term basis.  The services available include: homemaking, personal care, skilled nursing care, and care management.  The program helps persons who require assistance to remain in their homes.

 

PASCOFIX

Funds are available through the PascoFix program for repairs to your home (except mobile homes) to make it decent and livable.  Repairs may include but are not limited to roof replacement, A/C and appliance replacement, electrical and plumbing repairs .The maximum after-rehab value of the property cannot exceed $200,000.   West Pasco: 727-862-9291, East Pasco: 353-523-1500.

 

SENIOR HEALTH CLINIC

The CARES Senior Health Clinic provides low income and uninsured seniors 55 years and older with limited medical care such as general physical exams, limited primary treatment, monitoring of chronic illnesses, counseling, and referrals.  Some donated medications, lab services and other specialty services also can be offered.  A staff of volunteer active and retired health professionals provides clinic services.

 

SHINE (SERVING HEALTH INSURANCE NEEDS OF ELDERS)

This Area Agency on Aging health insurance assistance program for elder Floridians provides educational materials and free unbiased insurance counseling to elders, caregivers and family members.  SHINE 1-800-963-5337

 

SKILLED MEDICARE SERVICES

CARES Home Health provides skilled home care to Medicare beneficiaries who meet eligibility requirements, who are homebound and who are under the care of a physician.  Services available include skilled nursing care, physical therapy, occupational therapy, speech therapy, medical social services, and home health aides.  These services are not intended for the person who needs chronic custodial care.
